FormXen

FormXen is a web form generating tool that provides a simple, user friendly interface to create online surveys, feedback forms, job applications, event registrations, and more.  Data results are tabulated and summarized in both graphical and spreadsheet formats.  Users can then quickly export the results directly to their computer.

 Reporting and Exporting
In addition to allowing you to custom-build your own forms, FormXen includes a custom reporting module with filters and data displays defined per survey or form.  The data can then be exported in spreadsheet format to use in other applications.
