Form Design and Creation: Create unlimited customized forms with FormXen's rich-text editor interface, which allows you to specify formatting details, build tables, upload your own images, and more. Forms can also be configured so that users can upload an attachment with their submission.

Question Formatting: FormXen features multiple question formats including short answer, long answer, radio group (select one), check box (select many), and dropdowns. Each allows you to customize values or use predefined responses for quick survey creation.

Look & Feel: FormXen also allows you to create themes and templates that you can apply to your forms for added standardization and a professional look and feel.

| Submission Management |

| Your forms can be published or unpublished depending on your needs. FormXen collects and stores the responses to your forms as they are submitted. Old and out-of-date submissions can be deleted either one by one, or all at the same time. Your system administrators can log in and review all the submissions, or filter to review them based on certain criteria. |

Publishing Forms: You can publish your forms, making them available for submission, or you can unpublish them, making them both unavailable and invisible to anyone who visits your site. This is helpful in instances such as rebuilding a form, or when you want to only utilize the form during a certain time of the year.

|
Form Submission: Your forms can be linked to your web site, so visitors to the site can easily find them and fill them out. Additionally, depending on each form's configuration, users can add attachments to their submission such as a picture or a Word document.

|
|
Delete Submissions by Date: FormXen allows you to delete submissions by date, so that all the forms submitted before the date you enter will be deleted.
|

|
Submission Review: Review all the form submissions for a given form, or filter the submissions based on your specified criteria. Notes can be added to the submission while going through it. Any documents attached to the form can also be downloaded and reviewed.

| Reporting and Data Exporting |

| FormXen provides a variety of ways to view your form submissions. It includes comprehensive reporting functionality, with custom reports designed using the filter criteria you specify. You can also export all the submissions of a form to an Excel document on your machine for review and comparison later. |

Customized Filtering Reports: AllofE can create one or more customized reports to help you view your submissions more efficiently. You can pick any question or set of questions in any of your forms, and we will build a report that searches the submissions according to the response to those questions, and displays all the required data in a readable format. You can run this report anytime, with whatever search criteria fit your needs.

Customized Data Summary Reports: Summary reports are used for statistical analysis, such as quickly displaying the results of an opinion survey to show which of a selection of items most users liked best.

|
|
Data Exporting: For each form on your system you can either view individual submissions, or you can click the Export button and download all the submissions to your machine for later review. This is also helpful if you want to send submissions to a colleague.

| Security and Permissions |

| FormXen includes a robust security structure based on levels and permissions, and AllofE will work with you to configure the permissions in your system to your organization's requirements. As well as customized levels and user groups, individual permissions can be assigned based on which forms pertain to which departments, and even which users can view a form. |

Levels: AllofE will customize your security structure to fit your organization. Users will all be assigned a user level, and each level will have permissions set for the functionality that type of user can access.
|
|
|
|
User Groups: User groups allow selected access to topic-specific material. For instance, users in the athletic department at a school district, and users in the theatre department, could be members of separate department-specific groups so that each department only sees the forms related to them.
|
|
|
|
Permissions: Administrators in your system can set permissions for specific forms, form submissions, and pages, so that users have access only to the information that they can view and update.
